    1. Pixie-16 model and serial number

    2. Host operating system and version

    3. Pixie SDK version

    4. Broadcom PCI/PCIe SDK version (PLX)

    5. Crate controller and PCI/PCIe bridge model numbers

    6. Crate serial number

  3. Description of the problem and steps to re-create

  4. Supporting data in the form of Pixie SDK log file with DEBUG log level

  5. Other information you find relevant
